STEP 1: Request a Free Consult

Complete the Request for a Free Consult Form. An appointment will be set up for a 15 minute consult to ascertain if a behavior plan is appropriate for your child at this time. If a behavior plan is determined as an appropriate next step for your child, then you will move on to STEP 2.

STEP 2: Make a Payment
|
|
A payment of $49.99 needs to be made prior to services rendered. Payment may be made via PayPal. Payment through PayPal is safe and simple. If you do not have a PayPal account it is free and only requires your email address and the registration of a bank account.
There are several payment options with PayPal. PayPal passes its hefty fees for the debit and credit card options on to ADD Coach Services. Consequently, payment by check via PayPal, is the only method ADD Coach Services accepts at this time. PayPal's check processing fee is minimal and will be absorbed by ADD Coach Services.

After paying PayPal complete the Behavior Plan Request Form. Please return to this page after making your payment by using your back browser. Click on BPRF to be taken to the form.

STEP 4: Thirty Minute Behavior Plan Consult
|
|
When ADD Coach receives your completed Behavior Plan Request Form, you will receive an email notifying you of your 30 minute consultation appointment. Once you send a return email accepting the appointment time, you will receive an email regarding the preparation for the consultation.
By the end of the behavior plan consultation, you will have your family's customized behavior plan!
